Job Title:

Physical Therapist Home Health

Pay and Typical Schedule:

Clinicians are paid per completed visit and documentation. CareCo offers a guaranteed pay program for therapists who agree to work a set number of days each week and accept all assigned visits. There is also a per diem option paying by the visit within a defined geographical area.

Position Description:

Work independently providing care to patients in their homes. Collaborate with nurses and other disciplines to provide care, education, and support to patients and caregivers under the supervision of the Clinical Director.

Qualifications:

• Graduate of an approved physical therapy school (American Physical Therapy Association, Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council on Medical Education of the AMA in collaboration with the APTA)
• Registration in Connecticut
• Home health experience and familiarity with OASIS are helpful
