BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a full-time opportunity providing home health visits. This position services adult and geriatric clients across Lancaster and Lebanon County.

As a home care Physical Therapist, you will be a vital part of a multidisciplinary healthcare team, delivering rehabilitative care and skilled nursing to help clients remain at home.

Responsibilities include:

1. Make home visits to clients in designated areas.
2. Perform diagnostic tests such as mobility, joint range, ambulation patterns, strength, endurance, balance, and safety assessments.
3. Implement wound care interventions using evidence-based dressings and topical agents.
4. Develop and modify individualized care plans, including manual therapy, gait training, and balance exercises.
5. Assess and update care plans regularly, collaborating with nursing staff as needed.
6. Educate clients and their families on rehabilitative activities to promote health, safety, and independence.
7. Document all observations, interventions, and evaluations accurately using electronic tablets.

Qualifications:

• Current Pennsylvania Physical Therapist (PT) license.
• Graduation from an American Physical Therapy Association-approved program, evidenced by diploma or transcript.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with electronic medical records (EMR) preferred.
