Job Title: Physical Therapist - Home Health

Job Description: As a Physical Therapist in our client's Home Health team, you will be responsible for providing Physical Therapy services to patients in their homes. Your duties include assessing patient functioning levels, developing treatment programs to improve independence and physical/cognitive functioning, and educating patients and caregivers. You will also supervise staff and report on patient progress.

Key Responsibilities:

• Perform assessments and develop treatment plans
• Record and report patient reactions and condition changes
• Instruct patients and caregivers on therapy programs and device use
• Supervise physical therapy assistants and certified nursing assistants when appropriate
• Prepare reports and participate in staff and agency development activities

Travel: Local travel to residential homes.
