Job summary

The Employee Connect is seeking a licensed Physical Therapist to join our Home Health team in Sterling Heights, Michigan. You will evaluate patients, develop individualized treatment plans, and deliver skilled PT services in patients' homes, fostering autonomy and strong patient relationships.

Qualified candidates hold a Michigan PT license, a Doctor of Physical Therapy degree, and at least 1 year of clinical experience (home health preferred).

Qualifications

• Licensed Michigan Physical Therapist in good standing.
• Doctor of Physical Therapy degree from an accredited program.
• Minimum of 1 year of clinical experience (home health preferred).
• Valid driver's license and reliable transportation.
• Strong knowledge of Medicare home health regulations and documentation standards.
• Proficiency with electronic medical records systems.
• Excellent interpersonal and communication skills with a patient-first mindset.

Responsibilities

Physical Therapist - Home Health

Location: Sterling Heights, MI

Pay Rate: $75,000 to $95,000 per year

Key Responsibilities

• Conduct thorough initial evaluations and develop individualized treatment plans for patients recovering from surgery, injury, stroke, or managing chronic conditions
• Deliver skilled physical therapy services in patients' private residences throughout the Sterling Heights and surrounding communities
• Establish measurable, functional goals in collaboration with patients and their families
• Monitor and document patient progress accurately and in compliance with all regulatory requirements
• Communicate regularly with referring physicians and the broader care team to ensure coordinated, high-quality care
• Educate patients and caregivers on home exercise programs, safety strategies, and fall prevention techniques
• Manage a caseload efficiently while maintaining excellent standards of clinical care and documentation
• Participate in case conferences and contribute to ongoing quality improvement initiatives

Requirements

• Active Michigan Physical Therapist license in good standing
• Graduate degree from an accredited physical therapy program
• Minimum of one year of clinical experience, preferably in home health or a related setting
• Valid driver's license and reliable personal transportation
• Strong working knowledge of Medicare home health regulations and documentation standards
• Demonstrated ability to work independently with excellent time management and organizational skills
• Proficiency with electronic medical records systems
• Outstanding interpersonal and communication skills with a patient-first mindset

Preferred Qualifications

• Experience in a related field is preferred
