Height Talent Solutions is seeking a dedicated Physical Therapist for Home Health across Pennsylvania and Michigan. You will provide skilled PT services in patients' homes, collaborating with a clinical team to deliver high-quality outcomes.

This full-time role offers independent work, a flexible schedule, and a pay-per-visit compensation structure with travel to local patient homes. The successful candidate will have a PT license in PA or MI and at least 1 year of healthcare experience.

Qualifications

• Licensed PT in PA or MI per state requirements.
• Bachelor's or Master's degree in Physical Therapy from an accredited program.
• Minimum 1 year healthcare experience; new grads may be considered.
• Valid driver's license and reliable transportation.
• Experience with EMR documentation; HomeCare HomeBase a plus.

Responsibilities

• Deliver skilled physical therapy services in residential home settings.
• Develop and implement individualized care plans with the care team.
• Complete clinical documentation accurately and on time using EMR.
• Travel locally to patient homes; approximately 20% driving.

Compensation: Pay Per Visit (PPV) — rates based on visit type and home health experience

About the Role

A well-established home health and hospice organization is seeking a compassionate Physical Therapist to join its care team across multiple territories in Pennsylvania and Michigan. This is a full-time, community-based role for PTs who enjoy working independently, building relationships with patients in their homes, and being part of a collaborative clinical team that genuinely cares about outcomes.

Key Responsibilities

• Deliver skilled physical therapy services to patients in residential home settings
• Develop and implement individualized care plans in coordination with the broader care team
• Complete clinical documentation accurately and on time using EMR software
• Manage your own schedule and caseload within the assigned territory
• Travel locally to patient homes — approximately 20% of your day is driving

Qualifications

• Bachelor's or Master's degree in Physical Therapy from an accredited program
• Active PT license in the state where you'll be working (PA or MI)
• Minimum 1 year of healthcare experience (new grads may be considered)
• Valid driver's license and reliable transportation

• Ability to self-manage time, schedule, and meet charting deadlines
