Job summary

Crossroads Home Health and Hospice is seeking a dedicated Physical Therapist to join our home health team. You will assess patients in their homes, designing individualized treatment plans to improve strength, mobility, and independence.

You will collaborate with physicians and care staff to deliver evidence-based therapies, educate patients and families on exercises, monitor progress, and adjust plans to maximize outcomes in the comfort of the patient's home.

Qualifications

• Current state license as a Physical Therapist.
• At least 1 year of experience in home health or a related field.
• Strong knowledge of anatomy, physiology, and kinesiology.

Responsibilities

• Conduct thorough evaluations of patients' physical conditions and develop individualized treatment plans.
