Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, stamina, pain, and other medical conditions. Based on this, the therapist develops treatment plans, performs interventions to improve patient well-being, and evaluates progress.

• Evaluate the patient's functional status and needs, assisting in developing the therapy plan of care with the physician
• Monitor and report on patient responses and condition changes to the healthcare team
• Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to standards of practice
• Educate patients, families, and healthcare team members about the plan of care, limitations, and progress

Education and Experience

Licensure Requirements

• Current Physical Therapy licensure in Indiana
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle
