BAYADA Home Health Care

is currently seeking an experienced Physical Therapist, PT for full-time, part-time, or per diem opportunities performing home health visits for our York office, located in York, PA. This office services adult and geriatric clients across territories throughout Adams County.

Compensation: $5,000 Sign-On Bonus for Full-Time!

As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing, enabling clients to receive necessary medical care while remaining at home.

Responsibilities for a PT - Physical Therapist include:

1. Make home visits to clients in designated geographic territories.
2. Perform diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
3. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
4. Assess and revise the care plan regularly, collaborating with nursing in a multidisciplinary team as appropriate.
5. Educate clients, family members, or representatives on rehabilitative care and activities to promote health, safety, and independence.
6. Accurately document observations, interventions, and evaluations using a state-of-the-art touchpad tablet.

Qualifications for a PT - Physical Therapist include:

1. Current PA Physical Therapist (PT) license.
2. Graduation from an American Physical Therapy Association-approved program or the Committee on Allied Health Education and Accreditation of the American Medical Association, evidenced by transcript or diploma.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Solid computer skills; prior experience with electronic medical records (EMR) preferred.
