Job description

BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for full-time, part-time, or per diem opportunities performing home health visits for our York office, located in York, PA. This office services adult and geriatric clients on a per-visit basis in territories throughout Adams County.

As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to remain at home with appropriate medical support.

Responsibilities for a PT - Physical Therapist include:

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as mobility and range of joints, transfer status, stability, ambulation patterns and appearance, muscle strength and endurance, balance testing, and safety assessments.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continuously assess and revise the care plan, collaborating with nursing in the multidisciplinary team as appropriate.
• Educate clients, family members, or representatives on rehabilitative care and activities to promote health, safety, and independence.
• Document observations, interventions, and evaluations accurately using a state-of-the-art touch pad tablet.

Qualifications for a PT - Physical Therapist include:

• Current PA Physical Therapist (PT) license.
• Graduation from an American Physical Therapy Association or CAAHEP-accredited program, evidenced by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency in computer skills; prior EMR experience preferred.
