Responsibilities

PHYSICAL THERAPIST - Home Health & Hospice

Join CHI Home Health and Hospice to experience a fulfilling and rewarding career. Provide compassionate, personalized care in patients' homes, helping those recovering from injuries, living with chronic illnesses, or with other medical needs.

The Physical Therapist evaluates and treats disabilities, injuries, and diseases using manual therapy, modalities, and exercises. Responsibilities include patient education, consulting, charting, supervision, and training of staff. The role serves diverse populations, including pediatric, medical/surgical, orthopedics, outpatient, and homecare, with potential increased travel.

Essential Key Job Responsibilities

1. Evaluate and treat patients referred by providers
2. Supervise Physical Therapist Assistants and support staff
3. Complete documentation promptly
4. Provide educational in-services for staff and providers
5. Collaborate with healthcare team and offer professional recommendations
6. Participate in care coordination activities
7. Promote the department and profession within the community
8. Perform other duties as assigned

Qualifications

Requirements

• Degree from an accredited baccalaureate, Masters, or Doctorate in physical therapy
• Licensed to practice as a physical therapist in ND
• At least two years of relevant experience
• Current CPR certification
• Valid ND driver’s license

Preferred

• Experience in community or home health
