Responsibilities

PHYSICAL THERAPIST - Home Health & Hospice

Join CHI Home Health and Hospice to experience a fulfilling, rewarding, and inspiring work environment. Provide compassionate, personalized care in patients' homes, helping those recovering from injuries, living with chronic illnesses, or managing other medical needs.

The Physical Therapist evaluates and treats disabilities, injuries, and diseases using manual therapy, modalities, and exercises. Responsibilities include patient education, consulting services, compliance with standards, charting, supervision, and training of staff. The role serves diverse populations, including pediatric, medical/surgical, orthopedics, outpatient, and homecare, which may involve increased travel.

Essential Key Job Responsibilities

1. Evaluate and treat patients referred by providers.
2. Supervise Physical Therapist Assistants and support staff.
3. Complete timely documentation of patient care.
4. Conduct educational in-services for staff and providers.
5. Collaborate with healthcare teams and provide professional recommendations.
6. Participate in care coordination activities such as care conferences.
7. Promote the department and profession positively within the hospital, community, and beyond.
8. Perform other duties as assigned by management.

*This position offers up to a $15,000 sign-on bonus.*

Qualifications

Requirements

• Degree from an accredited baccalaureate, master's, or doctorate program in physical therapy.
• Licensed to practice as a physical therapist in ND.
• At least two years of relevant experience.
• Current CPR certification.
• Valid ND driver’s license.

Preferred

• Experience in community or home health settings.
