Benchmark Therapy and Rehab is seeking a Physical Therapist for home care, offering both full-time and PRN schedules. You will evaluate and treat patients in a home setting, develop treatment plans, and collaborate with physicians and other care team members to improve mobility and reduce pain.

Candidate must hold state licensure to practice physical therapy. Mileage reimbursement is provided as part of the role.

• Evaluates and treats patients recovering from injury, disease, or surgery in a home care environment.
• Establishes treatment plans and treats patients using exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility.
• Works cooperatively with physicians, case managers and treating therapists.
• Monitors, documents, and reports patients' progress and communicates needed health information regarding the patient to all appropriate health care team members.
• Reports to manager of clinical services.
