Benchmark Therapy and Rehab is seeking a Physical Therapist for home care in the Dayton, Ohio area. The role involves evaluating patients at home, developing individualized treatment plans and applying hands-on therapies to improve mobility and reduce pain.

You will work closely with physicians and care teams, documenting progress and adjusting plans as needed. A state PT license is required to practice in this setting, with flexible scheduling and mileage reimbursement available.

Physical Therapist- Home Care - Full Time and PRN

Requirements and Responsibilities:

• Evaluates and treats patients recovering from injury, disease, or surgery in a home care environment.
• Establishes treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility.
• Works cooperatively with physicians, case managers and treating therapists.
• Monitors, documents, and reports patients' progress and communicates needed health information regarding the patient to all appropriate health care team members.
• Requires state license to practice physical therapy.
• Reports to manager of clinical services.
