Job summary

Plan A Health is seeking a dedicated Physical Therapist to support patients in their homes, helping regain mobility, independence, and confidence. You will assess functional status, craft personalized plans, and deliver hands-on therapy while coordinating with the care team to ensure seamless transitions.

Ideal candidates have DPT or Master’s in PT, Texas PT licensure, 1–2 years of home health experience, and CPR certification.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the practice state
• 1–2 years of experience, preferably in home health
• CPR certification

Responsibilities

• Evaluating and assessing patients’ functional status, mobility, and rehabilitation potential
• Developing and implementing individualized care plans in collaboration with the care team
• Providing hands‑on therapy to improve strength, balance, and independence
• Educating patients and caregivers on treatment strategies and safety
• Monitoring progress, adjusting treatment plans, and supporting safe care transitions
• Collaborating with nurses, physicians, and other clinicians to ensure coordinated care
• Documenting care and participating in team meetings
• Supervising Physical Therapist Assistants and guiding caregivers
