Job summary

SYDIERA Healthcare's Home Health team is seeking a dedicated Home Health Physical Therapist to join our compassionate care network. You will collaborate with an interdisciplinary team to develop and implement personalized care plans that help patients achieve their goals in the comfort of their homes.

Qualifications

Bachelor's or Master's degree in Physical Therapy from an accredited program.
Current Physical Therapist license in the state of practice.
At least one year of healthcare experience.

Experience using tablets, smartphones, and EMR systems.

Responsibilities
