Job summary

Pride Health is seeking a Physical Therapist (Home Health) to support a healthcare facility in Charlotte, NC. The position runs Monday–Friday, 8:00 AM–4:30 PM, with a local contract and a travel-friendly caseload.

You will perform evaluations, develop and implement care plans, deliver skilled interventions, and document in EMR while coordinating with physicians and the inter-disciplinary team. Mileage reimbursement applies; CMS and agency policies must be followed.

Qualifications

• Active Physical Therapist license required.
• CPR certification required.
• At least 1 year of home health experience; familiarity with HCHB and OASIS is a plus.

Responsibilities

• Perform comprehensive PT evaluations and ongoing reassessments.
• Develop and adjust individualized care plans to improve mobility and independence.
• Deliver interventions including therapeutic exercise, gait, balance, and education.
• Document evaluations, visits, progress, and discharge summaries in EMR.
• Coordinate care with physicians and the interdisciplinary team.
• Communicate changes in patient condition and barriers to progress promptly.
• Ensure CMS, infection control, and agency policy compliance.
• Manage a mobile caseload and maintain productivity and documentation.
• Adapt to new environments and patient populations as a travel clinician.

Pride Health is hiring a Physical Therapist (Home Health)tosupport our client's healthcarefacility in Charlotte, NC.

Job Details:

Schedule: Monday-Friday | 8:00 AM-4:30 PM
Location: Charlotte, NC

Coverage Area: Mecklenberg and Union County

Contract Length: 15+ Weeks Contract (Possibility to Extend)

Job Type: Local Contract

Pay Range: $56.00 - $60.00/hour (W-2) Plus Mileage reimbursement

Mileage Reimbursement Rate: $0.52/ mile

*Pay offered is based on experience,expertise, credentialing, and education

Duties:

• Perform comprehensive physical therapy evaluations and ongoing reassessments to identify impairments, functional limitations, and safety risks.
• Develop, implement, and adjust individualized care plans to improve mobility, strength, balance, endurance, pain management, and functional independence.
• Deliver skilled interventions including therapeutic exercise, gait training, transfer training, balance activities, stair training, and home safety education.
• Educate patients and caregivers on home exercise programs, fall prevention, assistive device use, and energy conservation techniques.
• Complete accurate and timely documentation (evaluations, visit notes, progress reports, and discharge summaries) using EMR systems.
• Coordinate care with physicians and interdisciplinary team members, including nurses, occupational therapists, speech therapists, and home health aides.
• Communicate changes in patient condition, safety concerns, or barriers to progress promptly to the care team.
• Participate in case conferences, care coordination activities, and team collaboration.
• Ensure compliance with CMS regulations, infection control standards, and agency policies.
• Manage a mobile caseload efficiently while meeting productivity and documentation expectations.
• Adapt quickly to new environments, workflows, and patient populations as a travel clinician.

Requirements:

• License(s): Active Physical Therapist license,CPR Certification
• Years of experience: At least 1 year of home health experience and experience with HomeCare HomeBase (HCHB) &OASIS is a plus
