Job summary

Dedicated Home Health is seeking a Per Diem Home Health Physical Therapist (PT) to evaluate patients in their homes across Palm Beach County, with Medicare-certified Home Health experience preferred.

Join our experienced clinical and operations team, enjoy flexible patient visits, and help patients regain strength, mobility, safety, and independence at home.

Qualifications

• Current Florida Physical Therapist license.
• Medicare-certified Home Health experience.
• Experience completing home health evaluations, reassessments and discharges.
• Strong clinical documentation and care coordination skills.
• Reliable transportation and ability to travel within the assigned territory.
• Commitment to timely, patient-centered care.

Responsibilities

• Evaluate patients at home, develop individualized plans of care, provide skilled physical therapy interventions, assess progress and coordinate with physicians, nursing and other members of the interdisciplinary care team.
• Deliver excellent clinical care while helping patients remain safe, mobile and independent at home.

What We're Looking For

Required:

• Current Florida Physical Therapist license
• Medicare-certified Home Health experience
