Interim HealthCare of West Palm Beach is seeking a PRN Home Health Physical Therapist to deliver one-on-one therapy in patients' homes in Palm Beach, FL. You will build mobility, strength, and independence through individualized treatment plans and direct patient interaction.

Qualifications

• Graduate of an accredited Physical Therapist Program and active PT license in FL
• Minimum of 1 year of physical therapy experience in home healthcare
• CPR certification and knowledge of state and federal home health regulations
• Good clinical judgment, strong interpersonal skills, dexterity, and compassion

Responsibilities

• Provide home-based physical therapy to patients with mobility, strength, and functional deficits
• Collaborate with home health team (RN, LPN, CNA, HHA, OT, SLP) to implement the plan of care
• Observe movements, conduct assessments, document progress and modify care plan as needed
• Assist patients with exercises and hands-on therapy to manage pain and improve mobility
• Educate patients and families on the plan of care, goals, exercises, and self-care

As a Home Health Physical Therapist, here’s what to expect:

• Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT, and SLP, focused on the patient’s plan of care and goals
• Observe movements, conduct assessments, document progress and modify care plan as needed
• Assist patient with exercises and hands‑on therapy to manage pain and increase mobility
• Educate patients on their plan of care, established goals, exercises, and self‑care
• Coach family on the plan of care, therapy goals and safely assisting with exercises

A few requirements for Home Health Physical Therapists:

• Graduate of an accredited Physical Therapist Program and active PT license in FL
• Minimum of 1 year of physical therapy experience in home healthcare
