Qualifications

• Current Florida Physical Therapist license.
• Medicare-certified Home Health experience.
• Experience completing home health evaluations, reassessments and discharges.
• Strong clinical documentation and care coordination skills.
• Reliable transportation and ability to travel within the assigned territory.
• Commitment to timely, patient-centered care.

Responsibilities

• Evaluate patients in their homes and develop individualized plans of care.
• Provide skilled physical therapy interventions and assess progress.
