Job summary
Dedicated Home Health is seeking a Physical Therapist (PT) for per diem home health visits in Palm Beach County, Florida. You will evaluate patients in their homes, develop individualized plans of care and provide skilled PT interventions to improve mobility and independence.
Ideal candidates hold a Florida PT license with Medicare-certified home health experience, strong documentation and care coordination skills, and reliable transportation for travel within the territory.

As a Per Diem Home Health Physical Therapist, you will evaluate patients in their homes, develop individualized plans of care, provide skilled physical therapy interventions, assess progress and coordinate with physicians, nursing and other members of the interdisciplinary care team.
