Dedicated Home Health is seeking a Per Diem Home Health Physical Therapist in Florida to join our team. You will evaluate patients in their homes, develop individualized plans of care, and provide skilled physical therapy interventions on a per-diem basis.

The role offers flexible visits and requires Medicare Home Health experience, with opportunities to collaborate with an experienced clinical and operations team.

Qualifications

• Current Florida Physical Therapist license.
• Medicare-certified Home Health experience.
• Experience completing home health evaluations, reassessments and discharges.
• Strong clinical documentation and care coordination skills.
• Reliable transportation and ability to travel within the assigned territory.
• Commitment to timely, patient-centered care.

Responsibilities

• Evaluate patients in their homes and develop individualized plans of care.
• Provide skilled physical therapy interventions and assess progress.
• Coordinate with physicians, nursing and other team members to ensure care.
• Maintain accurate documentation and ensure timely discharges when appropriate.

The Role
