Job summary

Preferred Care Home Health is hiring a full-time Physical Therapist to provide in-home orthopedic and mobility-focused therapy across Palm Beach County. You will evaluate patients, develop and implement individualized treatment plans, and monitor progress to achieve independence in daily activities.

You will educate patients and caregivers, document outcomes, and collaborate with physicians and the care team to optimize recovery. A valid PT license and reliable transportation are required.

Qualifications

• Degree from an accredited Physical Therapy program.
• Active, unrestricted Physical Therapist license in good standing.
• Home health experience preferred but not required.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide skilled therapy services in accordance with physician orders and established care plans.
• Monitor patient progress and adjust interventions to support optimal outcomes.
• Educate patients and caregivers on therapeutic exercises, safety techniques, and the use of assistive equipment.
• Complete accurate and timely clinical documentation.
• Communicate changes in patient condition to physicians and the interdisciplinary care team.

What You’ll Do
