Erickson Senior Living’s Oak Crest Village is seeking a resident-centered Physical Therapist to support residents on our award-winning campus in Parkville, MD. This role works within our Home Health/Home Care setting to provide personalized care across the continuum.

The position emphasizes documentation accuracy, adherence to Medicare/state requirements, and collaboration with an interdisciplinary team to ensure high-quality outcomes for older adults.

Qualifications

• Must possess a valid and current state Physical Therapy license.
• Experience with OASIS documentation system required.
• Therapeutic experience in older adults across multiple diagnostic groups is preferred.
• Experience and the ability to complete documentation to meet reimbursement and regulatory requirements.

Responsibilities

• Provide a holistic approach, supporting residents' physical, spiritual, emotional, and clinical needs.
• Implement residents' personal care plans, focusing on strengths, preferences, and routines.
• Collaborate with the interdisciplinary clinical team to address resident needs throughout care.

Location: Oak Crest Village by Erickson Senior Living

We are hiring a resident-centered Physical Therapist to join our team. All of our residents live within our award-winning campus, creating an environment where you can be part of their journey throughout their continuum of care.

Our Physical Therapists support our residents in all aspects of daily living, providing person-centered care to ensure we meet all of our resident’s needs.

We are seeking candidates who specifically have experience within Home Health / Home Care fields.

Schedule: Flex/PRN arrangement (daytime hours).

Compensation: $55.00 hourly.

• Providing a holistic approach, supporting the residents' physical, spiritual, emotional, and clinical needs.
• Implementing residents’ personal care plans, focusing on residents’ strengths, preferences, and preferred routines, while providing a sense of comfort, companionship, and belonging.
• Working with the interdisciplinary clinical team to ensure the needs of our residents are being addressed throughout their care.
