Job summary

Erickson Senior Living is seeking a resident-centered Physical Therapist to join Oak Crest Village in Parkville, MD. The role emphasizes supporting residents in daily living with person-centered care and collaboration with an interdisciplinary team.

Flex/PRN daytime schedule is offered. Must hold a valid state PT license and have experience in home health/home care settings to address residents' needs across diagnostic groups.

Qualifications

• Must have a Physical Therapy degree and meet Medicare/state requirements.
• Experience with OASIS documentation system is required.
• Therapeutic experience with older adults across multiple diagnostic groups is preferred.
• Ability to document to meet reimbursement and regulatory requirements.

Responsibilities

• Provide resident-centered physical therapy and support daily living goals.
• Collaborate with interdisciplinary clinical team to address residents' needs.
• Deliver care in accordance with Medicare/state requirements and documentation standards.

Oak Crest Village by Erickson Senior Living We are hiring a resident-centered Physical Therapist to join our team. All of our residents live within our award-winning campus, creating an environment where you can be part of their journey throughout their continuum of care.

Our Physical Therapists support our residents in all aspects of daily living, providing person-centered care to ensure we meet all of our resident's needs.

We are seeking candidates who specifically have experience within Home Health / Home Care fields.

Schedule:

Flex/PRN arrangement (daytime hours).

Compensation:

$55.00 hourly.

• Providing a holistic approach, supporting the residents' physical, spiritual, emotional, and clinical needs.
• Implementing residents' personal care plans, focusing on residents' strengths, preferences, and preferred routines, while providing a sense of comfort, companionship, and belonging.
• Working with the interdisciplinary clinical team to ensure the needs of our residents are being addressed throughout their care.

What we require:

• Degree and training must meet current Medicare and state requirements.
• Must possess a valid and current state Physical Therapy license.

• Experience with OASIS documentation system required.
