Qualifications

• Must have valid state Physical Therapy license.
• Experience in Home Health/Home Care.
• Experience with OASIS documentation system is required.
• Geriatric physical therapy experience preferred.

Responsibilities

• Provide resident-centered physical therapy services.
• Develop and implement individualized care plans.
• Collaborate with interdisciplinary clinical team to address residents' needs.
