Responsibilities include evaluating patients, developing treatment plans, documenting progress, coordinating with physicians, and ensuring proper coding and billing for services rendered. Growth opportunities and ongoing staff education are available.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license and/or registration in the state (as applicable).

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD orders.
• Document findings on the standardized evaluation form, accurately and adhering to guidelines.
• Develop a comprehensive treatment plan with long/short-term goals, frequency, duration, and modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Deliver comprehensive treatment using modalities and adjust as indicated while following precautions.
• Document daily participation and progress on weekly progress notes, timely.
• Update treatment plan and document discharge/in-services as indicated.
• Co-sign notes as required; supervise junior staff or students as needed.
