Reliant Rehabilitation is seeking a licensed Physical Therapist to deliver comprehensive therapy services in skilled nursing facilities. You will assess patients, develop treatment plans, and coordinate care with facilities to optimize outcomes.

You will supervise PTAs and Rehab Techs, maintain compliance with state and federal regulations, and participate in continuing education to advance clinical skills within Reliant Rehabilitation’s framework.

Formación

• Graduate of an APTA-accredited school of physical therapy.
• Candidate must hold a valid physical therapy license in the state.
• Experience providing therapy in skilled nursing or similar setting is preferred.

Responsabilidades

• Provide physical therapy and related services to patients in SNF settings.
• Develop, implement, and update initial and ongoing treatment plans of care.
• Discharge planning and coordination with facilities and clients.
