Job summary

Concept Rehab, Inc seeks a licensed Physical Therapist to provide competent evaluation and treatment for mobility and functional needs. You will collaborate with physicians and healthcare teams, supervise staff, and contribute to ongoing education and training within a supportive environment.

The PT reports to the Therapy Program Manager and works to uphold Concept Rehab's commitment to excellence in rehabilitative care while maintaining professional documentation and patient-centered care.

Qualifications

• Licensed in the state(s) where services are being provided as a Physical Therapist.
• Doctoral/Graduate degree in Physical Therapy (DPT) or equivalent.

Responsibilities

• Evaluate patients for rehabilitation within 24–48 hours of physician orders.
• Develop effective treatment plans and implement them.
• Treat patients according to the plan of care.
• Supervise the PT/PTA staff delivering clinical services.
• Communicate progress with physicians, managers, patients and families.
• Record treatment notes, progress notes and daily billing; discharge planning.
• Engage in in-services and staff training.
• Assist with equipment recommendations and clinical program development.
