Qualifications

• Evaluate patients for rehabilitation services within 24 to 48 hours of physician’s orders.
• Develop effective treatment plans.
• Communicate with physicians and clinical managers regarding patient progress.

Responsibilities

• Supervise the Licensed Physical Therapy Assistant in delivering clinical services.
• Record treatment and progress notes per policy and procedure.
• Participate in discharge planning.
