Concept Rehab is seeking a Physical Therapist to provide evaluation and treatment for mobility, function, and activity limitations. You will collaborate with families, physicians and the health team, documenting services and supporting ongoing education for staff.

The successful candidate will supervise a PT assistant, participate in care planning and interdisciplinary meetings, and contribute to maintaining high professional standards and the CRI mission of commitment, respect, and integrity.

Competenze

• Licensed Physical Therapist with current license in the state(s) where services are provided.
• Active engagement in rehabilitation settings and collaboration with health teams.

Mansioni

• Evaluate patients for rehabilitation services within 24 to 48 hours of physician orders.
• Develop effective treatment plans.
• Treat patients according to the treatment plan of care.
• Supervise the Licensed Physical Therapy Assistant delivering clinical services.
• Communicate with physicians, managers, and health team members about patient progress.
• Participate in Plan of Care and Interdisciplinary Team Meetings.
• Meet productivity standards established by CRI.
• Lead in-service training for facility staff.
• Record treatment notes and discharge summaries; handle billing per policy.
• Educate families/facilities on maintenance programs.
• Participate in discharge planning.
• Recommend needed equipment to improve care delivery.
• Report equipment issues to facility staff.
• Act as clinical supervisor for affiliate PT programs.
