Lifepoint Rehabilitation is seeking a Physical Therapist (PT) for Canyon Vista Medical Center in Sierra Vista, AZ. This full-time role focuses on delivering evidence-based physical therapy across multiple care settings, with active involvement in discharge planning and patient education.

The ideal candidate holds an accredited PT degree, a current PT license in Arizona, and a valid BLS certification, with rehab or acute care experience preferred.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapy (PT) license in state of practice.
• Basic Life Support (BLS) certification required.

Responsibilities

• Evaluates, plans, and delivers evidence-based PT interventions across hospital, rehabilitation, inpatient, medical-surgical and outpatient settings.
• Performs comprehensive assessments and develops individualized treatment plans.
• Collaborates with interdisciplinary team and participates in discharge planning.
