Job summary

Luxor at Sayville Rehabilitation and Nursing Center is seeking an experienced Physical Therapist (PT) to assess referred patients and provide direct care in a long-term care setting. The PT will establish treatment plans, set goals, document patient progress, and educate patients and families on therapy techniques.

The role requires graduation from an accredited PT program, current licensure, and experience supervising PTAs.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Must hold a valid PT license/registration for state of practice.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term care or geriatric care preferred.
• Strong documentation skills.
• Documentation of evaluations, progress notes, discharge summaries.
• Collaborates with disciplines to provide patient-centered care.
• Punctual, professional, HIPAA compliance.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals based on patient capacity.
• Document and record patient condition and educate patients and families.
• Oversee and supervise Physical Therapy Assistants.
• Collaborate with other disciplines to provide best patient-centered care.
