Job summary

Luxor at Sayville Rehabilitation and Nursing Center is seeking an Experienced PT for our Facility. The Physical Therapist will assess referred patients, develop and administer treatment plans, and educate patients and families on therapy methods.

The role requires graduation from an accredited PT program, current license, and the ability to supervise Physical Therapy Assistants. Prior long-term care or geriatric care experience is preferred, with strong documentation and teamwork across

Qualifications

• Must have graduated from an accredited Physical Therapy program.
• Must hold a valid professional license to practice in the state.
• Experience supervising Physical Therapy Assistants preferred.

Responsibilities

• Assess referred patients and establish treatment goals based on capacity and tolerance.
• Administer therapy and monitor progress toward goals as directed by the physician.
• Document patient conditions and educate patients/families on therapy methods.
• Collaborate with other disciplines to deliver patient-centered care.
• Maintain timely documentation: evaluations, progress notes, and discharge summaries.
• Ensure confidentiality and compliance with HIPAA and Residents' Rights.
