Job summary

Sayville Rehabilitation & Nursing Center (Luxor at Sayville) in New York is seeking an Experienced Physical Therapist to join our facility. The PT will assess referred patients, develop and administer treatment programs, and collaborate with the physician to meet functional goals.

Responsibilities include supervising Physical Therapy Assistants, documenting progress, and educating patients and families on therapy methods.

Qualifications

• Graduate from an accredited Physical Therapy program and licensed in the state.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term care or geriatric settings preferred.
• Strong documentation and record-keeping skills.

Responsibilities

• Assess referred patients and determine rehabilitation needs.
• Develop and administer treatment plans with goals.
• Document patient condition and progress regularly.
• Educate patients and families on physical therapy methods.
• Collaborate with other disciplines to provide patient-centered care.
• Report to work on time and maintain professionalism.
