Job summary

Luxor at Sayville Rehabilitation and Nursing Center is actively seeking an experienced Physical Therapist to join our facility and lead patient rehabilitation efforts.

You will assess patients, develop and implement treatment plans, document progress, and educate patients and families while supervising Physical Therapy Assistants in a long-term care setting. Licensure and compliant documentation are required.

Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Must hold a valid professional license and registration for the state of practice.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing, evaluations, progress notes, recertification and discharge documentation.
• Collaborates with all disciplines to provide patient-centered treatment and care.
• Compliance with confidentiality, HIPAA and Residents’ Rights.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct PT care.
• Establish and administer a treatment program with goals based on patient capacity and tolerance.
• Document and educate patients and families in an appropriate physical therapy method.
• Oversee and supervise Physical Therapy Assistants.
• Maintain confidentiality and adhere to HIPAA and residents’ rights.
• Conduct evaluations, weekly progress notes, recertifications and discharge documentation.
• Collaborate with all disciplines to deliver best patient-centered care.
