Luxor at Sayville Rehabilitation and Nursing Center is seeking an Experienced Physical Therapist (PT) for our facility. The PT will assess referred patients, provide direct care, and establish goal-directed treatment programs under physician direction.

The role includes supervising PT assistants, coordinating with interdisciplinary teams, and documenting patient progress while ensuring confidentiality and high-quality patient care in a long-term care setting.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Valid state professional license and registration.
• Experience supervising Physical Therapy Assistants.
• Geriatric or long-term care experience preferred.
• Strong documentation writing skills.

Responsibilities

• Assess referred patients to determine rehab needs and potential.
• Provide direct patient care and implement treatment plans.
• Establish goals based on patient capacity and tolerance under physician direction.
• Document patient status and progress; educate patients and families.
