Job summary

Woodstock Nursing & Rehabilitation Center in Woodstock, Georgia is seeking a passionate Physical Therapist to join our team. You will work with a diverse patient population to restore strength, mobility and independence, developing and implementing individualized treatment plans.

You will perform assessments, collaborate with an interdisciplinary team, provide interventions (exercises, manual therapy, modalities), educate patients and families, and maintain detailed records to track progress.

Qualifications

• Bachelor's or Master's degree in Physical Therapy from an accredited program.
• Minimum of 0 years of experience as a Physical Therapist ( licensure or certification pending)
• Possession of a valid Physical Therapy license in the state of Georgia

Responsibilities

• Conduct patient evaluations and assessments to identify treatment needs and goals
• Develop and implement individualized treatment plans to promote optimal functional outcomes
• Provide physical therapy interventions, including but not limited to: exercises, manual therapy, and modalities
• Contribute to the development and implementation of patient education programs
• Communicate effectively with patients, families, and healthcare team members
• Maintain accurate and detailed records of patient treatment and progress
