Job summary

Vitruvian Health in the region of northwest Georgia is seeking a licensed Physical Therapist to provide in-home physical therapy services to geriatric patients through Hamilton Medical Center Home Health. You will evaluate, plan, and implement treatment in patients’ homes under physician guidance.

This full-time role requires Georgia licensure, a valid driver’s license, and the ability to work independently with a compassionate, patient-centered approach.

Qualifications

• Graduate of an accredited college of physical therapy.
• Current Georgia license as a Registered Physical Therapist.
• 1 year of clinical care or public health experience.
• CPR certification and ability to document patient care clearly.

Responsibilities

• Evaluate, plan, and implement physical therapy for homebound patients.
• Provide geriatric-focused PT services for Hamilton Home Health.
• Consult with physicians to ensure treatment plans meet requirements.
