LTC Therapy Services is seeking a Physical Therapist (PT) for a full-time opportunity at Vantage Health and Rehab of Wakefield in Wakefield, MA. You will help residents improve mobility and safety through targeted therapy, whether recovering from a hospital stay or managing a chronic condition.

You will assess, set measurable goals, adjust plans as needs change, document care, and collaborate with the interdisciplinary team.

Qualifications

• Active Massachusetts Physical Therapist license required.
• Strong clinical judgment and documentation skills expected.

Responsibilities

• Assess residents' mobility and establish measurable treatment goals.
• Provide therapy addressing walking, transfers, strength, and balance.
• Adjust treatment plans as residents improve or their needs change.
