Summary

We are hiring a FT Physical Therapist for home health visits in Sarasota!

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist helps determine a treatment plan, performs interventions to improve the patient's well-being, and evaluates progress.

Responsibilities

• Evaluate patients' functional status and physical therapy needs; assist in developing the therapy plan of care with physicians
• Monitor and report patient responses to treatment and any condition changes to the supervising nurse or physician
• Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to standards of practice
• Educate healthcare team members, patients, and families about the plan of care, functional limitations, and progress

Education and Experience

Licensure Requirements

• Current Physical Therapy license in Florida
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle
