What You Will Do

• Practice independently in the community as part of an interdisciplinary care team.
• Conduct comprehensive assessments of patients, determine needs, and develop treatment plans and expected outcomes.
• Deliver personalized rehabilitation therapy to patients in their homes or care facilities.
• Evaluate evolving patient needs and adjust plans of care accordingly.

Qualifications

• Minimum of one-year physical therapy experience or successful completion of probation as a VNS Health Rehab Intern
• New York State License and current registration in Physical Therapy
• Valid driver's license or NYS Non-Driver photo ID card may be required

Pay Range per Visit

• For the per diem per visit position, the range is $70- $75 per visit.

Pay Range

USD $85,000.00 - USD $127,500.00 /Yr.
