Qualifications

• Valid driver's license required for areas outside Manhattan.

What You Will Do

• Practice independently in the community as part of an interdisciplinary care team
• Conduct comprehensive assessments of patients to determine needs and develop treatment plans with expected outcomes
• Deliver personalized rehabilitation therapy in patients' homes or care facilities
• Evaluate patient progress continuously and adjust plans of care accordingly

Qualifications

• Minimum of one-year physical therapy experience or successful completion of probation as a VNS Health Rehab Intern
• License to practice as a Physical Therapist in New York State
• Valid driver's license required for areas outside Manhattan

Pay Range per Visit

For all areas other than Bronx applying to the per diem per visit position: $70 - $75 per visit.

For Bronx applicants: $90 - $95 per visit.

Salary Range

USD $85,000.00 - USD $127,500.00 per year
