The physical therapist is responsible for providing physical therapy services and for adherence to all conditions outlined in the personnel services agreement.
Responsibilities
- Assessing and evaluating therapeutic, rehabilitative, functional status and participation in the development of the total plan of care. Evaluates home environment.
- Assesses for muscle strength, mobility, gait, ROM – potential for rehab.
- Directing physical therapy treatment.
- Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
- May train patient in the use of prosthetic device.
- Provides physical therapy consultation to home families/caregivers when indicated and provides in-service education programs for nursing organization personnel as needed.
- When therapy is only skilled service, instructs, supervises and evaluates home health aide care every two weeks.
- Participates in discharge planning for patient and in peer consultation process.
Education And Work Experience
- Required Degree: Bachelor’s Degree in Physical Therapy approved by an accredited organization.
- Preferred Degree: Master’s Degree in Physical Therapy approved by an accredited organization.
- Certificate(s): PT License and CPR certification, if required by state
- Experience: 2+ Years

- Ability to show compassion and understanding.
- Knowledge in HIPAA laws and confidentiality policies.
- Ability to create and implement treatment plans based on patients’ specific condition and needs.
- Ability to evaluate, conduct and develop customized treatment plans based on findings.
- Strong observation, time management, detail orientation, communication and interpersonal skills.
Physical Requirements
- Ability to stand, sit, talk, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard.
- Ability to lift, carry, push, and/or pull light to moderate amounts of weight up to 100 lbs.
- Ability to operate various equipment based on department.
- The ability to hear, understand, and distinguish speech and/or other sounds (e.g., machinery alarms, medical codes or alarms).
Clarity Of Vision
- For near visual acuity, working with small objects or reading small print, including use of computers (approximately 20 inches or less).
- For far visual acuity, 20 feet or more. Ability to recognize features as well as see a person or object.
- For peripheral vision, observing an area that can be seen up and down or to right or left while eyes are fixed on a given point.

Compensation And Benefits
- Compensation Pay Range: $80,591.00 - $102,456.00 per year
- Reasonable Pay Estimate: A reasonable estimate of the pay range for this position is $80,591.00 - $102,456.00 per year
