Join us!*The physical therapist is responsible for proving physical therapy services and for adherence to all conditions outlined in the personnel services agreement.**ESSENTIAL RESPONSIBILITIES:*** Assessing and evaluating therapeutic, rehabilitative, functional status and participation in the development of the total plan of care. Evaluates home environment.* Assesses for muscle strength, mobility, gait, ROM – potential for rehab.* Directing physical therapy treatment.* Initiates physical therapy program and instructs other personnel and/or family /caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.* May train patient in the use of prosthetic device.* Participates in discharge planning for patient and in peer consultation process.* Provides physical therapy consultation to home families/caregivers when indicated and provides Inservice education programs for nursing organization personnel as needed.* When therapy is only skilled service, instructs, supervises and evaluates home health aide care every two weeks.**EDUCATION AND WORK EXPERIENCE:*** Required Degree: Bachelor’s Degree in Physical Therapy approved by an accredited organization.* Preferred Degree: Master’s Degree in Physical Therapy approved by an accredited organization.* Certificate(s): PT License and CPR certification, if required by state* Experience: 2+ Years**KNOWLEDGE, SKILLS, AND ABILITY:*** Ability to show compassion and understanding.* Knowledge in HIPAA laws and confidentiality policies.* Ability to create and implement treatment plans based on patients’ specific condition and needs.* Ability to evaluate, conduct and development customized treatment plans based on findings.* Strong observation, time management, detail orientation, communication and interpersonal skills.**PHYSICAL REQUIREMENTS:*** Ability to stand, sit, talk, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard.* Ability to lift, carry, push, and/or pull light to moderate amounts of weight up to 100 lbs.* Ability to operate various equipment base on department.* The ability to hear, understand, and distinguish speech and/or other sounds (e.g., machinery alarms, medical codes or alarms. Four hearing requirements listed: In person speech, Telephone, Other remote speech and other sounds.Clarity of Vision:* For near visual acuity Clarity of vision at approximately 20 inches or less (i.e., working with small objects or reading small print), including use of computers.* For far visual acuity, Clarity of vision at 20 feet or more.
