Job summary

MEP Health is seeking a Physical Therapist (PT) to join a home health care team, providing services that enable patients to recover at home in Groveland, FL. You will deliver therapy under physician orders and coordinate with the care team to support positive outcomes.

Responsibilities include assessments, treatment plan development, progress documentation, and discharge planning. A current FL PT license and CPR certification are required, with opportunities to supervise PTAs/HHAs as appropriate.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a PT.
• Valid CPR certification.
