Job summary

Award Care Health Group is seeking a Physical Therapist to join our family‑oriented team in Florida. The role focuses on planning, implementing and evaluating physical therapy services, with emphasis on resident care and restorative programs.

Qualifications

• Graduate of School of Physical Therapy.
• Minimum of three months recent experience as a Physical Therapist in a nursing home environment preferred.
• Licensed to practice physical therapy in Florida; CPR certification preferred.

Responsibilities

• Assist in planning, developing, organizing, implementing and directing physical therapy services and programs.
• Interpret policies and procedures to staff, residents and families as necessary.
• Participate in discharge planning and development of resident care plans and assessments.
• Initiate restorative programs in line with residents' care plans.
• Document changes and follow up on patient condition.

Major Responsibilities

• Assist in planning, developing, organizing, implementing, evaluation and direction our physical therapy service as well as it programs and activities as direct.
• Assist in interpreting physical therapy policies and procedures to personnel residents, family members, etc. as necessary.
• Participate in discharge planning, development and implementation of resident care plans, resident assessment.
• Initiate individual restorative program for residents in conjunction with the residents' plan of care.
• Brief resident of procedures involved in physical therapy and prepare necessary equipment.
• Reassure resident before and during therapy treatment.
• Be alert to resident' response to treatment. Notify therapist immediately for assistance if required.
• Assist residents in ambulatory training as directed.
• Demonstrate to resident and nursing personnel, as necessary, the use of medical appliances involved in physical therapy. (e.g., wheelchairs, crutches, canes, braces, and prosthetic)
• Create an environment and a culture that enables the facility to fulfill its mission.
• Must document changes and follow up on condition.

Qualifications

Education:

• Graduate of School of Physical Therapy

Experience:

• Minimum of three months recent experience as a Physical Therapist working in a nursing home environment preferred.

Licenses/Certifications:

• State of Florida Physical Therapy License
• CPR Certification
