Job summary

Sunset-Lake Health and Rehabilitation Center is seeking a licensed Registered Physical Therapist to join our rehab team in a skilled nursing setting. We offer a new wage scale and a supportive, team‑oriented environment to recognize your expertise.

You will plan and oversee therapy services, collaborate with nursing and medical staff, and develop individualized care plans while maintaining accurate documentation and resident communication. Full-time, in-person role.

Qualifications

• Must be licensed as a Registered Physical Therapist in good standing.
• Experience in skilled nursing or long-term care preferred.
• Strong leadership and communication skills.

Responsibilities

• Lead physical therapy services and develop care plans.
• Coordinate with nursing and medical staff to deliver therapy.
• Monitor resident progress and document findings.

Responsibilities for a Physical Therapist Include:

• Plan, organize, and lead our Physical Therapy services in accordance with all federal, state, and local regulations
• Collaborate with nursing, medical staff, and other departments to coordinate high-quality therapy care
• Develop individualized care plans, implement therapy programs, and monitor resident progress
• Participate in quality assurance and policy development
• Maintain accurate records, documentation, and billing in compliance with standards
• Provide education and communication to residents, families, and staff Requirements:
