Job summary

FutureCare in Maryland is seeking a licensed Physical Therapist to evaluate, diagnose and treat movement dysfunction, restoring mobility and independence for residents. Qualifications

• Bachelor of Science in Physical Therapy; licensure in Maryland or eligible.
• Ability to develop and implement comprehensive care plans.
• Knowledge of accreditation standards and compliance requirements.

Responsibilities

• Review resident information to assess impact on treatment.
• Conduct and document a thorough functional assessment.
• Develop and implement individualized treatment plans with achievable goals.
• Comply with facility policies, practice regulations and state/federal rules.
• Complete re-evaluations per regulations; review PT assistants’ interventions.
• Supervise and instruct rehab staff in procedures.
• Document treatments in the resident’s medical record after each session.
• Revise plans as needed throughout treatment.
• Prepare concise discharge summaries using prescribed formats.
• Orient residents to services and inform about procedure risks.
• Collaborate with nursing staff, residents and families to maximize function.
• Instruct residents and families on exercise programs to continue post-discharge.
• Assist in referrals for additional services after discharge.

Responsibilities

• Review available resident information and evaluates the impact of this information on the assessment and treatment process
• Conduct and document a thorough functional assessment of the resident, utilizing standardized procedures. Assessment of home/vocational and equipment needs is included
• Develop and implements an individualized treatment plan based on resident needs and containing achievable functional goals
• Complie with evaluation, treatment and documentation guidelines as defined by facility policy, practice regulations and state/federal regulations
• Complete re-evaluations according to practice regulations including review of physical therapy assistants treatment interventions, if appropriate
• Instruct assistants and rehab techs in treatment procedures to be utilized and provides appropriate supervision while such staff are working with residents
• Complete appropriate documentation in the resident’s medical record following each treatment session
• Revise treatment plans, as needed, throughout the treatment process
• Document the course of treatment in a clear, concise discharge summary utilizing a prescribed format
• Orient residents to program services and treatment procedures appropriate to their needs. Inform residents of any potential risk during any procedures
• Work with nursing staff, resident and families to maximize resident’s use of functional abilities
• Instruct residents and family/other caregiver about exercise programs and therapeutic procedures to be continued by the resident following treatment
• Assist in the referral process when a resident requires additional services following discharge. Maintain current knowledge of community resources

Qualifications

• Minimum of Bachelor of Science degree in Physical Therapy
• Current license or eligibility for licensure in the state of Maryland
• Minimum of 1 year of Long Term Care Experience preferred
• Certification and demonstrated competency in identified areas of specialization
• Demonstrated competency in applying the principles, methods, materials and equipment used in physical therapy
• Ability to assess resident needs and develop and implement a comprehensive plan of care
• Knowledge of accreditation standards and compliance requirements
