Job summary

FutureCare in Baltimore is seeking a Physical Therapist to evaluate and treat movement dysfunction, restore mobility, and create individualized plans promoting independence. The role involves direct resident care in a long-term care setting, supervision of staff, and collaboration with nursing and families.

Qualifications include a BS in Physical Therapy and Maryland licensure, with 1 year LTAC experience preferred. A comprehensive benefits package and career growth opportunities are offered.

Qualifications

• Bachelor of Science in Physical Therapy is required.
• Current Maryland license or eligibility for licensure.
• Minimum 1 year of Long Term Care experience preferred.
• Certification and demonstrated competency in identified areas of specialization.
• Ability to assess resident needs and develop a comprehensive plan of care.
• Knowledge of accreditation standards and compliance requirements.

Responsibilities

• Review available resident information and evaluate its impact on assessment and treatment.
• Conduct and document a thorough functional assessment using standardized procedures.
• Develop and implement an individualized treatment plan with achievable goals.
• Comply with evaluation, treatment and documentation guidelines per policy and regulations.
• Complete re-evaluations as required and supervise assistants when appropriate.
• Provide supervision to rehab staff and document progress in records.
• Document course of treatment and discharge summaries clearly.
• Orient residents to services and inform them of any risks in procedures.
• Collaborate with nursing, resident and families to maximize function.
• Instruct residents and families on exercises to continue post-discharge.
• Assist referral processes for additional services after discharge.

Responsibilities

• Review available resident information and evaluates the impact of this information on the assessment and treatment process
• Conduct and document a thorough functional assessment of the resident, utilizing standardized procedures. Assessment of home/vocational and equipment needs is included
• Develop and implements an individualized treatment plan based on resident needs and containing achievable functional goals
• Complie with evaluation, treatment and documentation guidelines as defined by facility policy, practice regulations and state/federal regulations
• Complete re-evaluations according to practice regulations including review of physical therapy assistants treatment interventions, if appropriate
• Instruct assistants and rehab techs in treatment procedures to be utilized and provides appropriate supervision while such staff are working with residents
• Complete appropriate documentation in the resident’s medical record following each treatment session
• Revise treatment plans, as needed, throughout the treatment process
• Document the course of treatment in a clear, concise discharge summary utilizing a prescribed format
• Orient residents to program services and treatment procedures appropriate to their needs. Inform residents of any potential risk during any procedures
• Work with nursing staff, resident and families to maximize resident’s use of functional abilities
• Instruct residents and family/other caregiver about exercise programs and therapeutic procedures to be continued by the resident following treatment
• Assist in the referral process when a resident requires additional services following discharge. Maintain current knowledge of community resources

Qualifications

• Minimum of Bachelor of Science degree in Physical Therapy
• Current license or eligibility for licensure in the state of Maryland
• Minimum of 1 year of Long Term Care Experience preferred
• Certification and demonstrated competency in identified areas of specialization
• Demonstrated competency in applying the principles, methods, materials and equipment used in physical therapy
• Ability to assess resident needs and develop and implement a comprehensive plan of care
• Knowledge of accreditation standards and compliance requirements
