Job summary

FutureCare Associates, Inc. is seeking a qualified Physical Therapist to evaluate, diagnose and treat movement dysfunction in residents.

The role focuses on developing individualized treatment plans and ensuring thorough documentation in the patient’s medical record. Applicants should hold a Bachelor of Science in Physical Therapy and be licensed or eligible for licensure in Maryland, with prior long-term care experience preferred.

Qualifications

  • Bachelor of Science degree in Physical Therapy required.
  • Current license or eligibility for licensure in Maryland.
  • 1 year of Long Term Care experience preferred.
  • Certification and demonstrated competency in identified areas of specialization.
  • Demonstrated competency in applying PT principles, methods, materials and equipment.
  • Ability to assess resident needs and develop comprehensive care plans.
  • Knowledge of accreditation standards and compliance requirements.

Responsibilities

  • Review resident information and assess impact on treatment.
  • Conduct and document thorough functional assessments.
  • Develop and implement individualized treatment plans with goals.
  • Complete evaluation, treatment and documentation per policy and regulations.
  • Perform re-evaluations as required by regulations.
  • Supervise rehab assistants and rehab techs during procedures.
  • Document treatment in the resident's medical record after each session.
  • Revise treatment plans as needed during treatment.
  • Prepare discharge summaries with clear documentation.
  • Orient residents to services and inform of any procedure risks.
  • Collaborate with nursing and families to maximize function.
  • Educate residents and families on home exercise programs.
  • Assist in referrals for additional services after discharge.

Overview

At FutureCare our Physical Therapists improve the quality of life for our residents. They show a deep sense of compassion and care by evaluating, diagnosing and treating movement dysfunction to restore mobility and create plans to get our residents moving toward independence and good health.

Salary Disclosure Statement

Salary Range $49.04 - $59.53/hr. Please note that the salary range has the potential to change and may be modified in the future, taking into consideration market conditions. The salary mentioned above reflects the potential base pay range for this role. Bonuses or other incentives (if applicable) are offered separately and paid pursuant to the relevant program schedule. All employment offers will consider such factors as overall experience, job-related qualifications, location, certifications/training, etc.

Responsibilities
  • Review available resident information and evaluate the impact of this information on the assessment and treatment process
  • Conduct and document a thorough functional assessment of the resident, utilizing standardized procedures. Assessment of home/vocational and equipment needs is included
  • Develop and implement an individualized treatment plan based on resident needs and containing achievable functional goals
  • Complete with evaluation, treatment and documentation guidelines as defined by facility policy, practice regulations and state/federal regulations
  • Complete re-evaluations according to practice regulations including review of physical therapy assistants treatment interventions, if appropriate
  • Instruct assistants and rehab techs in treatment procedures to be utilized and provides appropriate supervision while such staff are working with residents
  • Complete appropriate documentation in the resident’s medical record following each treatment session
  • Revise treatment plans, as needed, throughout the treatment process
  • Document the course of treatment in a clear, concise discharge summary utilizing a prescribed format
  • Orient residents to program services and treatment procedures appropriate to their needs. Inform residents of any potential risk during any procedures
  • Work with nursing staff, resident and families to maximize resident’s use of functional abilities
  • Instruct residents and family/other caregiver about exercise programs and therapeutic procedures to be continued by the resident following treatment
  • Assist in the referral process when a resident requires additional services following discharge. Maintain current knowledge of community resources
Qualifications
  • Minimum of Bachelor of Science degree in Physical Therapy
  • Current license or eligibility for licensure in the state of Maryland
  • Minimum of 1 year of Long Term Care Experience preferred
  • Certification and demonstrated competency in identified areas of specialization
  • Demonstrated competency in applying the principles, methods, materials and equipment used in physical therapy
  • Ability to assess resident needs and develop and implement a comprehensive plan of care
  • Knowledge of accreditation standards and compliance requirements
